Scheduling Your Surgery

The date and time of your surgical procedure and appointment for a pre-surgical screening will be scheduled by your physician’s office. If you need to change your pre-screening visit, please contact surgery scheduling at (954)962-3213. Any changes regarding the date or time of your surgery must be coordinated through your physician’s office.

A pre-screening visit to our Center will be arranged. It may be on the day of surgery or a prior day. During your pre-screening appointment, you will complete all the necessary admitting and insurance forms.  Please be sure to bring all insurance information with you.

You will then be directed to our diagnostic testing area to complete any pre-admission tests ordered by your physician. In some cases, these tests may be performed in your physician’s office prior to your pre-screening visit. In that event, the test results should be furnished to the Center prior to your pre-screening appointment, or you may bring them with you.

You will be asked to complete a surgical anesthesia evaluation form regarding your medical history and current physical status.  One of our team of board certified anesthesiologists will meet with you personally during your pre-screening visit or immediately prior to your surgery.
